New Client Packet PDF (Establish an Account): So you have decided to give our services a try? Great! We do need a bit of information from you first in order to setup a service account. This form may be completed and mailed or faxed in advance, or it can be given to the service representative prior to the start of work.; Rev. 3.1 (17-MAY-2005)
Work Authorization & Contract PDF: Are you planning to pay for our services by credit card and do not wish to establish a service account? It must be completed and returned to the service representative prior to the start of work. You must complete the form from scratch each time we provide on-site or depot service unless you establish a service account.; Rev. 1.0 (11-NOV-2005)
Credit Card Authorization PDF (Single Page): If you would like to pay by credit card and your order total, including shipping and taxes, is over $100 USD, then you will need to complete this one-page credit card authorization form. We will also need copies of the front and back side of the credit card you will be using. The form and copies of the card can be mailed, faxed, or given to your account representative.; Rev. 2.3 (11-NOV-2005)
Credit Card Authorization PDF (With Options): This longer credit card authorization form is designed for and may be used by larger companies that need to autorize additional signers, delivery addresses, or certain recurring orders. Sales & Use Tax Exemption Packet PDF: If you are exempt from sales/use tax, or if you will be purchasing items for resale, we must have a current copy of both your state-issued certificate and this multi-jurisdiction uniform sales and use tax certificate before we can waive taxes. You can mail, fax, or give the certificates to your account representative. At present, we only collect taxes on orders shipped to CA, CT, MA, and NV.; Rev. (JUL-2000)
